The crown looks fine but just doesn't fit!
Sometimes this can occur when the plastic impression tray is a bit too small
for the arch and or the patient may have bitten too hard. If you can see the
tray through the impression material(see fig#1 - triple tray and #2
quadrant) then the tray has possibly flexed. The flex can be really slight
so the crown appears fine but fits like it was made on a different die.
We have found a solution for this problem...... metal impression trays.
There are many options but we like a disposable tray made by Clinitions

Are contacts sometimes open or too tight?
If the contacts of a crown are perfect on the model and either too tight or
open in the mouth, then the teeth may have moved either because the temp was
a bit tight or loose or the model system the lab uses could be inaccurate.
The model system we use is expensive and extremely accurate, it's the most
like a solid model we've ever found.
When we adjust the contacts we check with .5 micron shim paper for the
highest accuracy.
